We discuss theories leading to the intermediate state $N\Delta$ and $\Delta \Delta$ contributions to $V_{\mathrm{NN}}$. We focus on the customary addition of ${\mathcal{L}}_{\Delta N\pi }^{\prime }$ to ${\mathcal{L}}_{\pi \mathrm{NN}}^{\prime }$ in a conventional field theory and argue that overcounting of contributions to $t_{\pi N}$ and $V_{\mathrm{NN}}$ will be the rule.
